Diabetes is at near epidemic levels in our community and rising. In response to this serious health threat, the Pajaro Valley Community Health Trust operates the Diabetes Health Center (DHC). This direct service program offers high-quality diabetes prevention/self-management education services and medical nutritional therapy in English and Spanish to all in our community who need them, regardless of their ability to pay for services.
Each year the DHC provides more than 1,500 individual patient visits with a Certified Diabetes Educator. Patients learn life changing skills that help them live a healthy lifestyle and prevent the more serious medical complications associated with uncontrolled diabetes. Over 40% of these services are provided to low-income, uninsured families free of charge.
We are committed to making diabetes services available to all community members, regardless of their financial or insurance status. Your donations help us continue the high quality services we offer to both children and adults in our community.
