The Diabetes Health Center (DHC), provides education and support for those living with diabetes or prediabetes and other health conditions. We strive to empower clients and provide tools to make informed decisions that improve quality of life and reduce the risk of complications from chronic medical conditions.

Diabetes Self-Management Education and Support

Learn how to manage your diabetes and related conditions. DSMES is tailored to you to meet and support your individual needs, goals and life experiences to improve your quality of life and reduce the risk of complications. DSMES includes our group classes and individual sessions.

Medical Nutrition Therapy

Certain medical conditions can be treated with changes in diet – this is where Medical Nutrition Therapy (MNT) comes in. 

MNT is designed for any individual living with acute and chronic conditions that can be improved or managed through simple lifestyle changes and healthier habits. This program is available for women, men, teens, children, and older adults.

The MNT Program is supported in part by grants from the Monterey Peninsula Foundation and Sutter Health.

NOTE: A physician referral including a diagnosis and any pertinent labs is needed for MNT services. We accept most insurances (including CCAH) for Diabetes and Nutrition related services. Any applicable copay amounts will be due at the time of service. A physician referral does not guarantee coverage. Please call your health plan for details about your coverage benefits. We offer reasonable self-pay rates for those who do not have insurance and for those whose insurance does not cover the services.

Group Classes

We’ll provide basic DSMES education and support in a group setting. Group classes are facilitated by our trained diabetes educator. Learn to live with diabetes and get ideas for success.

  • Monitor your glucose and interpret testing results
  • Manage your medications
  • Understand the connection between blood sugar and food
  • Strategize meal planning, healthy eating, and physical activity
  • Keep healthy and prevent complications

You can attend two 90-minute classes in person at the Diabetes Health Center (DHC), or virtually* in addition to your individual assessment with one of our educators. Call to register!

Workplace Wellness/ Community Presentations

Our trained educators can speak to groups of any size in English and Spanish.

Telehealth

Telehealth enables you to have a video visit with your dietitian or diabetes educator without leaving your home. You can use your smartphone, tablet or laptop to attend your appointment virtually.
