2009-2010 Healthy Lifestyles Grant Applications Are Now Being Accepted

The Community Health Trust is pleased to offer its Healthy Lifestyles RFP Grant Guidelines for prospective applicants.

Healthy Lifestyles RFP Grant Guidelines

All organizations interested in applying for a 2009-2010 Healthy Lifestyles grant will be expected to complete the Electronic Grant Application and Budget Worksheet. The completed application and budget should be emailed to grants@pvhealthtrust.org by December 11, 2009 at 5:00pm. All applicants are also required to submit the completed Grant Application Checklist and all required materials by the application due date. These additional materials can be submitted by mail, delivery, or email. Late or incomplete applications will not be considered for funding.

Budget Worksheet — Applicant organizations can use either the PDF or Excel version of the Budget Worksheet.It is recommended that applicants use the PDF version of the budget worksheet unless they need to add additional lines to the budget. Both versions have formulas that add up the totals.

The completed application, budget, grant application checklist, and all additional materials are due by 5:00pm Friday, December 11, 2009. The application should be emailed to

grants@pvhealthtrust.org.

This is the first year that the Community Health Trust has used an electronic grant application form.
