Application Process

Grants from the Health Trust are awarded annually through a competitive process to organizations that have programs or projects that advance the strategic goals in one or more of the Health Trust’s four program areas.  All organizations seeking a grant from the HealthTrust should apply through this process. Organizations may also request a hard copy of the grant guidelines.  It is important that applicant organizations read all instructions carefully to determine if their project addresses one of the specific strategic program area goals, and if the organization and project meet the eligibility criteria.

The Request for Proposals will be released in November. Please visit our website to view our grant guidelines and download application materials after this date.

Step 1

Letters of Intent (LOI) for the upcoming grant cycle are due in December. LOI’s must be received by 4:00 p.m. on the due date. The LOI must be submitted by email. After the LOI’s are reviewed, a formal proposal will be requested from those applicants whose projects best address the Health Trust’s program guidelines and priorities. Applicants will be provided with the necessary information for completing the application process at that time. Late or incomplete applications will not be considered for funding.

Step 2

Applicants will be notified if they are invited to submit a full proposal before the end of the year. If your agency has been invited to apply please download and complete the following documents: Grant Application, Scope of Work and Grant Request Budget. In addition to completing these documents please review the grant application check list found on the last page of the grant application and submit all additional required documentation. Please review the Definitions of Cultural Competency and Developing Measurable Outcomes before completing the application.
