Since 1999, the Diabetes Health Center (DHC) has been teaching people the skills they need for diabetes self-management. According to the American Diabetes Association, proper self-management of diabetes leads to reduced complications, improved coping skills for people living with diabetes, and improved quality of life. All services are offered in English and Spanish.
Mission
The Mission of the Diabetes Health Center is to prevent and reduce the incidence of acute and chronic complications associated with diabetes by providing culturally competent Diabetes Prevention and Self-Management Education and Medical Nutrition Therapy to residents of the greater Pajaro Valley area.
Philosophy
The DHC’s philosophy is to place diabetes clients at the center of a team of health care experts. Our highly skilled and caring professional team includes:
- A registered nurse certified to teach every aspect of diabetes self-management to help you learn how to better manage your diabetes and to avoid serious complications.
- A registered dietician certified to teach every aspect of diabetes self-management to help you create a plan that incorporates the foods you like to eat into a healthy balanced diet.
- Your physician who, in consultation with DHC staff, oversees your care and ensures that your medications are adjusted properly and assist you with planning and exercise program.
