Mission Statement

The Mission of the Diabetes Health Center is to prevent and reduce the incidence of complications associated with diabetes by providing culturally competent Diabetes Prevention and Self Management Education and Medical Nutrition Therapy to residents of the greater Pajaro Valley area.

General Information

United States Health and Human Services Secretary Tommy G. Thompson announced in November 2003, that the number of Americans with diabetes rose to an all-time high of an estimated 18.2 million people in 2003. This reflects a 1.2 million increase in people with diabetes in just two years. If diabetes remains untreated, it can lead to vision loss, heart disease, kidney disease and a number of other serious health problems. People with diabetes are fortunate because they can take steps to manage it effectively.

Studies show that staying within one's target blood glucose range, even half the time, can result in both immediate and long-term benefits.

Recent studies have shown that complications can be significantly delayed or prevented by tight control of blood sugar levels. The Diabetes Health Center Management Program is a multi-disciplinary program designed to meet the unique needs of the diverse population and those affected with diabetes in our community. This outpatient program will provide clients and their families with the tools needed for diabetes self-management. Proper self-management of diabetes leads to reduced complications, develops coping skills for living with diabetes and improves the quality of life for persons with diabetes.

El Secretario de Servicios Humanos y de Salud de los Estados Unidos, Tommy G. Thompson anuncio en Noviembre 2003, que el numero de personas con diabetes subio a 18.2 milliones de personas en el 2003. Esto refleja un aumento de 1.2 milliones de personas con diabetes en solamente dos años. Si la diabetes no es tratada puede llegar a causar problemas con la vista, corazón, riñones y varios problemas serios a la salud. Afortunadamente, laspersonas que tienen diabetes pueden tomar los pasos adecuados para controlar su diabetes efectivamente.

Los estudios indican que cuando se permanese en su meta de nivel de glucosa. Por lo menos la mitad del tiempo, puede obtener beneficios immediatos y a largo plazo.

El Centro de Diabetes es un programa de manejamiento multi-diciplinario, disenado para reunir las necesidades de una populacion diversa y aquellos afectados con diabetes en nuestra comunidad. Este programa les enseñara a los participantes y su familia los instrumentos necesarios para el manejamiento de su diabetes. Un buen manejo del diabetes lleva a reducir las complicaciones, y un buen plan de tratamiento medico reduce los costos relacionados con las hospitalizaciones. Y tambien promueve estrategias para vivir con la diabetes y mejora la calidad de la vida para las personas con diabetes.

Services

Individualized Education

Integrates lifestyle, medication, nutrition and exercise in teaching diabetes self-management education.

Self-monitoring Blood Glucose Training

Clients learn the differences between meters and are able to choose the appropriate meter that best suits their needs. Educator trains clients how to use, store and maintain meter.

Insulin Injection Training

Clients are instructed how to correctly draw up insulin, inject it and dispose of syringes, as well as, how to store insulin.

Nutrition Counseling

Clients are given the most current nutrition guidelines and may design an individualized meal plan incorporating food preferences, lifestyle and activity levels. Foods affecting blood sugar levels, food portion sizes and meal planning are included.

Group Classes

A comprehensive course covering the principles of diabetes self-management including disease process, medications, blood glucose monitoring, and signs, symptoms and treatment of hypo/hyperglycemia and prevention and delay of acute and chronic complications.

Educación individual

Integra el estilo de vida, medicamentos, la nutrición y el ejercicio en el plan del manejo de su diabetes.

Entrenamiento del manejo de glucosa (azucar) en la sangre

Los clientes aprenden la diferencia entre una maquina y otra para poder escojer la mas apropieda por si mismo, y la que llena sus necesidades. La enfermera entrenara al cliente como usar, mantener y como alsar su maquina.

Entrenamiento de como injectar Insulina

Al cliente se la dan las instrucciónes de como sacar la insulina correctamente, injectar y tambien como deshacerse de las jeringas y como almacenar su insulina.

Consejos de nutrición

Se les da a los clientes las guias de una buena nutrición y son referidos/a una Nutriciónista. Para planear sus comidas individuales incorporandolas en la preferencia de sus comidas del estilo de vida, y de sus actividades diarias.

Clase de grupo

Clases que enseñan los principios necesarios del manejo de su diabetes.
